Low Testosterone and Sleep Deprivation Linked to Psoriasis Risk in Men

An analysis of over 4,000 American men has revealed compelling evidence that low testosterone levels and inadequate sleep duration significantly increase the risk of developing psoriasis. The study, published in Clinical, Cosmetic, and Investigational Dermatology, utilized data from the National Health and Nutrition Examination Survey (NHANES) spanning 2011-2014.

Researchers analyzed data from 4,060 male participants, including 180 men diagnosed with psoriasis. The results demonstrated striking associations between two modifiable risk factors and psoriasis development:

Men with testosterone levels below 300 ng/dL showed a nearly threefold increased risk of psoriasis (OR=2.97, 95% CI: 2.16–4.09) compared to those with normal testosterone levels.

Participants sleeping fewer than seven hours per night had a 76% higher risk of developing psoriasis (OR=1.76, 95% CI: 1.33–2.35) compared to those getting adequate sleep.

The study’s authors explain that testosterone plays a crucial immunomodulatory role, helping regulate inflammatory responses in the body. When testosterone levels drop below normal thresholds, this can trigger increased systemic inflammation and compromise the body’s ability to control inflammatory cascades that contribute to psoriasis development.

Sleep deprivation compounds this effect by disrupting the hypothalamic-pituitary-adrenal axis, leading to elevated cortisol production and further systemic inflammation. The research revealed non-linear relationships, showing that extremely low testosterone levels plateaued in their risk impact, while sleep durations longer than eight hours provided diminishing protective benefits.

The findings suggest that both hormonal balance and adequate sleep represent potential intervention points for psoriasis prevention and management. The study’s authors note that the associations remained significant even after adjusting for multiple factors, including age, race, body mass index, smoking status, and existing health conditions.

Subgroup analyses revealed that these associations were particularly pronounced in men over 50 years of age and those with higher BMI, suggesting these populations may benefit most from targeted interventions addressing sleep hygiene and hormonal health.

While this cross-sectional study provides valuable insights, the researchers acknowledge limitations, including reliance on self-reported psoriasis diagnosis and sleep duration data. Additionally, the study’s design cannot establish causality, highlighting the need for longitudinal research to confirm these relationships.
